Job summary

In this role you will also manage adoptions and contribute to special programs and education, working collaboratively with staff to ensure smooth transitions and quality patient care.

Job description

Work Shift:

Day - 8 Hour (United States of America)

The Social Worker is responsible for completing bio psychological assessments of patients and development of patient care plans for post discharge services. Additional discharge planning activities include, but are not limited to referrals to home health, DME, dialysis, short-term rehabilitation, and nursing home placements. Also, services as a resource for staff, patients, and families with regard to community agencies. Manages adoptions and assists with special programs and education.
