Altus Hospice is seeking a dedicated Licensed Social Worker (LMSW) to deliver compassionate social work services to hospice patients and families in the San Antonio area. The role emphasizes counseling, care planning, and collaboration with the RN Case Manager and interdisciplinary team.

The ideal candidate has a Master of Social Work, licensure as a Social Worker, a valid driver's license, and at least one year of healthcare or hospice social work experience.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker by the State Board of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle.
• Minimum one year experience as a social worker in a healthcare setting; hospice experience preferred.
• State-specific requirement: active license in TX or listed states.

Responsibilities

• Assess psychosocial status and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Attend interdisciplinary meetings and address psychosocial stresses with the team.
• Provide empathetic counseling to patients and families.

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

Schedule: Per Diem, 5-10 per Month

Coverage Area: San Antonio and Surrounding Areas

Education

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumberedlicense as a Social Worker from the State Board of Social Work

$34.00 - $38.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
