Job summary

Alpha Omega Hospice is seeking a dedicated Licensed Medical Social Worker (MSW) to provide compassionate social work services to hospice patients and families in Carrollton, TX. This per diem role emphasizes counseling, emotional support, and care coordination within a multidisciplinary team.

The right candidate will hold a Master of Social Work, be licensed as a Social Worker in Texas, and have at least one year of healthcare experience; hospice experience is preferred.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker by the Texas State Board.
• Current Driver's License and access to a dependable vehicle.
• Minimum one year experience as a social worker in healthcare; hospice experience preferred.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement individualized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans and attend interdisciplinary meetings addressing psychosocial stresses.
• Provide empathetic counseling to patients and families.

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

TX: Current unencumbered license as a Social Worker from the State Board of Social Work.

$30.00 - $37.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
