Montefiore Health System is seeking a full-time Social Worker to provide comprehensive case management for high-risk patients. This role involves psychology assessments, crisis interventions, and connecting families with necessary physician services. Candidates must hold a valid NYS M.S.W. /L.C.S.W. license and be fluent in Spanish.

Associates are eligible to apply for a lateral transfer after completing one (1) year of service in their current position, with exceptions: an Associate who has received a written warning is not eligible for a transfer/promotion until six (6) months after the warning; an Associate who has been suspended is not eligible for transfer/promotion until one (1) year after suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department if such movement does not unreasonably reduce operational efficiency. Probationary periods are three (3) months for full-time employees and four (4) months for part-time employees working more than 3/5 of a normal workweek. Probation does not apply to shift changes.
This position is responsible for organizing and coordinating comprehensive social service case management for high-risk BHC patients and families, including developing and coordinating substance abuse treatment and domestic violence interventions. The Social Worker will provide supportive counseling, crisis intervention, assistance with concrete services/entitlements, linking families with medical sites, and securing appropriate referrals and linkages.
The role also involves conducting psychosocial assessments, clinical interventions, and developing case management services for individuals and families enrolled in the BHC. The position requires responding to diverse patient and family needs, provider demands, and ensuring contract compliance professionally. Work on special initiatives to meet grant deliverables is also expected.
Educational requirements include a NYS M.S.W. /L.C.S.W. license with at least one year of direct case management/social services experience in a community setting serving underserved populations. Experience with domestic violence case management is preferred. Spanish speaking ability is required.
