Job description

Overview

Provide comprehensive social work services, including assessment of a patient’s psychosocial functioning, determining needs for financial and medical assistance, offering supportive counseling, and arranging concrete services. Participate in counseling and evaluate the suitability of various treatment modalities. Coordinate interdisciplinary consultations.

Responsibilities

  • Evaluate patients within assigned areas or social work disciplines. Conduct ongoing assessments of social, psychological, environmental, and financial factors for DSRIP/BPCI-A patients.
  • Develop social work treatment plans and coordinate with multidisciplinary teams in program evaluation, development, and implementation of treatment and long-term plans.
  • Counsel patients, family members, significant others, and staff regarding adjustments to chronic illness, lifestyle changes, and treatment options.
  • Obtain and review educational materials; develop and distribute information as needed.
  • Represent patients to community agencies through referrals and follow-up. Ensure access to appropriate resources and develop relationships with external agencies.
  • Collaborate with other services to ensure proper and timely referrals for both permanent transfers and short-term transient care.
  • Monitor and supervise transportation referrals to ensure timely and appropriate services.
  • Communicate regularly with financial offices to exchange information and improve accounts receivable services.
  • Assist staff in reporting cases of patient abuse, neglect, or domestic violence.
  • Serve as a resource to staff, patients, families, and caregivers.
  • Perform other duties as assigned.

Qualifications

Education:

  • Master’s Degree from an accredited program by the Council on Social Work Education.
  • Complete coursework with specialization in clinical practice.

Experience:

  • No experience required with degree; internship or some experience in hospital or medical facility treatment preferred.

Knowledge and Skills:

  • Maintain current knowledge of treatment options, quality indicators, infection control, patient safety, patient rights, legal issues, reimbursement, billing, and receivables.
  • Strong oral and written communication skills; ability to develop and maintain professional relationships with patients, families, multidisciplinary teams, and external agencies.
  • Proficiency in utilizing electronic health record systems and billing systems relevant to this position.

Licenses, Certifications:

  • New York State Social Worker License.

Physical Requirements:

  • Prolonged standing, reaching, walking.
  • Ability to stoop, bend, lift, and carry up to 25 pounds.
