East Haven Nursing and Rehabilitation Center is seeking a dedicated Social Worker to join our compassionate team. You will work with residents, providing social, emotional, and psychological support to enhance well-being, and collaborate with our care team to ensure coordinated, person-centered care.

This full-time role focuses on assessments, care planning, counseling families, and addressing social determinants of health affecting seniors and individuals with disabilities in our facility.

Responsibilities

• Assess residents' social, emotional, and psychological needs and identify opportunities for improvement
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Provide counseling, guidance, and support to residents and their families
• Identify and address social determinants of health that impact residents' overall well-being
• Participate in family and resident meetings, as needed
• Maintain accurate and detailed records of resident interactions and care plans
