Job summary

Heritage Hills Rehabilitation and Healthcare Center in Rhode Island is seeking a part-time Social Worker to support residents and families with psychosocial needs, care planning, and coordination.

You will evaluate residents, develop individualized psychosocial care plans, provide counseling and referrals, and collaborate with interdisciplinary teams while ensuring compliance with facility policies.

Qualifications

• Flexible schedule required for part-time hours.
• Bachelor’s degree required; master’s preferred.
• Minimum 1+ years in healthcare social work; nursing home experience preferred.
• Strong knowledge of nursing facility regulations.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced, team-oriented environment.

Join our team at Heritage Hills Rehabilitation and Healthcare Center as a Social Worker.

Salary range $25 - $40/hr. (all inclusive)

Responsibilities for Social Worker:

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker:

• Must be able to work part-time hours and maintain a flexible schedule.
• Bachelor’s degree required; master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.
