Zusammenfassung

Brighton/Suncrest Hospice is seeking a dedicated Social Worker to assess and address the psychosocial, emotional, and spiritual needs of patients and families. You will coordinate with physicians, nurses, chaplains, and staff to create individualized plans of care and ensure timely support across transitions.

You will educate families about resources, advocate for patient access, and provide crisis intervention while maintaining compliant documentation and regulatory standards.

Qualifikationen

• Social Work degree compliant with CHAP and state regulations.
• Assess patients and families psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the plan of care.
• Counsel patients and families and provide emotional support, crisis intervention, and guidance during illness, loss, grief, and life transitions.
• Educate patients and families about community resources and connect them with appropriate programs and services.
• Advocate to identify barriers to care and develop solutions with the hospice team to meet patient goals.
• Participate in IDT meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other care team members.
• Maintain accurate, thorough documentation per hospice standards and regulatory requirements.
