Job Description

Purpose of Position
The Social Worker on the Adult In-Patient Unit is part of the Multi-Disciplinary Treatment team. They ensure compliance with departmental policies, conduct assessments, participate in treatment planning, and coordinate safe, timely discharge plans.

Duties & Responsibilities

• Conduct interviews with patients and/or family/significant others to determine ongoing care needs.
• Complete Social Work Comprehensive Assessment / Admission Note for all patients requiring social services, following departmental risk criteria.
• Ensure documentation meets social work and psychiatry department standards.
• Provide psychosocial information for patient transfers to ensure continuity of care.
• Offer supportive counseling, crisis intervention, and grief counseling as appropriate.
• Assist with patient adjustment issues, including depression, anxiety, denial, anger, and noncompliance.
• Develop and implement social work treatment and continuing care plans, documenting in medical records.
• Provide information, referrals, and advocacy for community resources.
• Coordinate interdisciplinary case conferences as needed.
• Supervise caseworkers and social work students when assigned.
• Lead or co-lead inpatient groups weekly.
• Identify and evaluate safety plans for victims of crimes or abuse.
• Maintain activity statistics and monitor documentation quality.
• Ensure compliance with all mandates and policies.
• Escort patients for placements when indicated.
• Participate in meetings, training, and cross-training programs.
• Coordinate discharge plans and respond to disaster situations as needed.
• Perform suicide risk assessments and manage related interventions.
• Review electronic medical records, perform shift handoffs, and utilize Psyches database.
• Perform other duties as assigned, demonstrating effective communication, documentation, prioritization, and understanding of psychosocial impacts of disease.

Minimum Qualifications

For Level II assignment, candidates must hold a valid LMSW or LCSW license issued by the NYSED.
