Job DescriptionThe Licensed Social Worker LVL II is an LMSW or an LCSW who works independently as well as in collaboration with transdisciplinary teams as a primary therapist who identifies psychosocial issues and concerns of patients and their families and provides clinical services to meet their needs. The Mental Health Continuum is a collaboration of various agencies, including the Department of Education. Through this collaboration, the Social Worker will support the behavioral health needs of students in the South Bronx.
Areas of ResponsibilityLicensed clinician with a minimum of 1 year of experience working clinically with individuals with mental health conditions and substance addiction who will be responsible for providing individual and group therapy to patients in the Child and Adolescent Outpatient Department of Behavioral Health. Candidate must have knowledge of resources and services for this population.
Summary Of Duties & ResponsibilitiesThe duties include functioning as a primary therapist for a subset of patients treated in the Child and Adolescent Outpatient Clinic, facilitating group therapy, conducting initial psychiatric assessments, documenting biopsychosocial assessments, formulating and dispositioning, developing comprehensive treatment plans, writing discharge summaries, and other related paperwork.
The therapist is responsible for facilitating supportive, psychoeducational, and insight-oriented psychotherapeutic interventions, providing crisis interventions, engaging with and providing support to families, and involving them in the process of recovery.
The candidate is responsible for identifying and collaborating with existing support systems, facilitating community referrals for patients as needed, collaborating with the interdisciplinary team to develop the most appropriate case dispositions, while documenting all interventions within the required regulatory timeframes. In collaboration with select schools, the Social Worker will provide timely mental health clinic appointments, urgent care, and tele-video visits to the students while in school or at home.
