We are seeking a Social Worker to join our team! This is a PRN / Per Diem position.

Hospital Details

VibraLife is a Rehabilitation Center and Senior Living Community in Mechanicsburg, Pennsylvania. It includes a short-term rehabilitation/skilled nursing facility and an assisted living unit, designed to address care and living needs post-injury or illness, offering a fully integrated care continuum.

Responsibilities

Provides patient assessment, counseling, and discharge planning. Acts as a liaison between patients, families, hospitals, and community resources. Works as part of an interdisciplinary team with physicians, nurses, and case managers. Uses specialized knowledge of social work, human development, and social systems to assist patients and families, including counseling and psychosocial interventions. Occasionally plans educational offerings and provides in-services.

Required Skills

• Bachelor’s Degree in Social Work
• Valid, active license to practice as a Social Worker in the state of employment

Additional Qualifications/Skills

• Master of Social Work preferred
