Complete Care at Maple Grove in Madison, WI seeks a Social Services Director (LSW/CSW) to lead resident advocacy and psychosocial assessments. You will direct MDS processes, coordinate transitions, and collaborate with families and the healthcare team to ensure dignity and holistic care.

You will oversee regulatory compliance, provide crisis support, and mentor staff while maintaining an audit-ready Social Services department.

Qualifications

• Bachelor’s Degree in Social Work (BSW) is required.
• Active LSW or CSW license in Wisconsin is required.
• 2+ years in LTC/SNF preferred; knowledge of Wisconsin healthcare landscape.

Responsibilities

• Lead psychosocial, mental, and emotional assessments for residents.

Strategic Responsibilities: Advocacy & Clinical Excellence

• Holistic Clinical Assessment: Lead the identification of psychosocial, mental, and emotional needs. You will move beyond the "Social History" to create a deep, narrative understanding of each resident's life and goals.
• Interdisciplinary Synthesis (MDS): Direct the Social Services portion of the MDS process, ensuring that psychosocial data is accurately captured to drive holistic, person-centered care plans.
• Proactive Discharge Navigation: Orchestrate complex transitions of care. You will serve as the master liaison between the facility and community resources, ensuring every resident has a "fail-safe" plan for their next chapter.
• Rights & Dignity Stewardship: Act as the facility’s lead advocate for resident autonomy. You will educate staff and families on resident rights, dignity, and self-determination, serving as a bulwark against neglect or exploitation.
• Crisis Intervention & Counseling: Provide expert emotional support and therapeutic communication for residents and families navigating the complexities of long-term care, cognitive decline, or end-of-life transitions.
• Regulatory Compliance: Ensure all documentation meets or exceeds stringent state and federal standards, maintaining an "audit-ready" department at all times.
• Community Networking: Cultivate relationships with Madison-area hospitals, VAs, and home health agencies to streamline resident support systems.

Professional Qualifications

• Education: Bachelor’s Degree in Social Work (BSW) is required.
• Licensure: Must hold an active, unencumbered LSW (Licensed Social Worker) or CSW (Certified Social Worker) license in the State of Wisconsin.
• Experience: 2+ years in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) is highly preferred. We are seeking someone who understands the nuances of the Wisconsin healthcare landscape.
• Technical Savvy: High proficiency in Electronic Health Records (EHR) and a solid grasp of MDS 3.0 requirements.
• Soft Skills: Exceptional emotional intelligence (EQ) and the ability to mediate complex family dynamics with grace and professionalism.
