Job summary

Genesis HealthCare's Oceanside Post Acute in Hampton, NH is seeking a Full Time Social Worker to help patients and families address social, emotional, and mental health needs, aiming to improve overall well-being and quality of life.

You will collaborate with the care team to create and implement care plans, assist with discharge planning, and connect families with community resources, within regulatory guidelines and company policies.

Qualifications

• Bachelor's degree in social work or human services is required.
• State licensure/certification as required by the state to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of an interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services.

Education

Bachelor's degree in social work or human services field
