Job summary

Oceanside Post Acute is seeking a Full Time Social Worker in Hampton, NH. You will collaborate with the care team to address social, emotional, and psychosocial needs of patients and families, and help coordinate services to enhance quality of life.

You will participate in care planning, conduct meetings with families, and manage discharge planning while building connections with community agencies. Bachelor’s degree in social work is required, with state licensure as needed for long-term care.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
