Job summary

Lubbock Family Medicine is seeking a Social Worker to identify and address the medically related social and emotional needs of patients. You will develop social histories and care plans, collaborate with physicians, and facilitate post-acute care transitions to support patients during and after stays.

Qualifications

• Bachelor's degree in social work or human services required.

Responsibilities

• Participates in interdisciplinary care plan conferences.
