Job summary

Cross Healthcare Services in Lakeway, TX, seeks a licensed Social Worker to support residents’ emotional and psychosocial needs and drive discharge planning in a multidisciplinary team.

You will assess, counsel, coordinate transitions, and help with advance directives while ensuring CMS and facility compliance to promote residents’ quality of life.

Qualifications

• Current Texas Social Worker license (LBSW/LMSW/LCSW) required.
• Bachelor's or Master’s degree in Social Work from an accredited program.
• Experience in skilled nursing, long-term care, rehab, or healthcare preferred.
• Knowledge of CMS regulations and long-term care standards preferred.
• Excellent communication, organizational, and interpersonal skills.
• Ability to maintain confidentiality and work effectively in a multidisciplinary team.

Responsibilities

• Complete psychosocial assessments and develop individualized care plans.

• Coordinate discharge planning and transitions of care.
• Assist with advance directives, community resources, and long-term care planning.
