Job summary

Cantex in Rosenberg, TX is seeking a Social Services Worker to identify and address the medically related social and emotional needs of patients and residents. You will provide individual and group social services and assist with discharge planning.

The role requires a bachelor’s degree in Social Work or a human services field, licensure or eligibility, strong communication and documentation skills, and at least one year of experience in long-term care or healthcare settings.

Qualifications

Responsibilities

• Develops social histories and care plans identifying psychosocial needs.

• Designs social service programs that meet patients' social and emotional needs per regulatory guidelines.
• Coordinates discharge planning with staff, patients, and families.
• Completes all required documentation and progress notes per state and federal regulations.
