Job summary

Blakeford at Green Hills in Nashville, TN seeks an experienced Social Worker to guide the resident experience in a long-term care environment. You will counsel residents and families, conduct regular visits, and coordinate social services to support transitions and discharge planning.

The role requires a bachelor’s degree in social work and at least one year of related experience, with a focus on dignity, independence, and compassion for seniors and their families.

Qualifications

• Bachelor’s degree in social work or related field required.
• At least 1 year of similar experience preferred.
• Ability to communicate across multiple generations with compassion, patience and inclusion.

Responsibilities

• Counsel residents and families on emotional, environmental or physical observations and interventions.
• Conduct regular face-to-face visits with assigned residents.
• Serve as resident liaison for social, health, and community services during discharge planning.
• Compile performance metrics and present summary data.

As a Blakeford Social Worker, you will:

• Counsel residents and their families on emotional, environmental or physical observations and interventions.
• Conduct regular face-to-face visits with assigned residents

• Compile performance metrics and present summary data
