Job summary

Manchester-Center-For-Rehabilitation-and-Healing-1 is seeking a Social Worker to join a Nashville-based nursing home team. The role focuses on supporting residents with daily living, psychosocial needs, and discharge planning.

Responsibilities

• Maintain/improve residents' ability to manage daily physical and psychosocial needs.
• Identify residents' social and emotional needs.
• Develop and maintain individualized care plans.
• Participate in admission, re-entry, or transfer planning.
• Attend interdisciplinary care plan meetings for social/emotional care.
• Coordinate with finance to educate residents and ensure proper documentation.
• Assist with discharge planning and arrange DME/home care for safe community return.

Social Worker

A nursing home in Nashville is seeking aSocial Worker to join their team.

Social Worker Job Description:

• Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Social Worker Qualifications/Requirements:

• BSW or MSW preferred, or Bachelors degree in the related field
