Job summary

St. Francis Hospital, The Heart Center, part of Catholic Health, seeks a Per Diem Social Worker to join our care team in New York. The LMSW provides psychosocial support, discharge planning, and connects patients and families with community resources.

Work with interdisciplinary teams to assess needs, educate on benefits and financial assistance, and address barriers to treatment. Requires NY LMSW license, MSW, and 1–3 years healthcare experience; SBIRT and NYS DOH screens certifications

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited school of social work.
• Current New York State Licensed Master Social Worker (LMSW) license.

Responsibilities

• Provide psychosocial assessments, counseling, and emotional support to patients and families.
• Assist patients and families with discharge and transitional planning, including coordination of post-acute services.
• Educate patients and families regarding available financial assistance programs, benefits, entitlements, and community resources.
• Identify and address psychosocial barriers that may impact a patient’s treatment, recovery, or discharge plan.
• Provide crisis intervention and utilize de-escalation techniques when needed.

We are looking for a Per Diem Social Worker to join our team!

The Licensed Masters of Social Worker (LMSW) provides comprehensive social work services to patients and their families, helping them navigate emotional, psychosocial, financial, and discharge-related needs. The LMSW collaborates with the interdisciplinary care team to assess patient needs, provide appropriate support and counseling, and connect patients and families with community resources.

Key Responsibilities

• Provide psychosocial assessments, counseling, and emotional support to patients and families.
• Assist patients and families with discharge and transitional planning, including coordination of appropriate post-acute services and community resources.
• Educate patients and families regarding available financial assistance programs, benefits, entitlements, and community resources.
• Identify and address psychosocial barriers that may impact a patient’s treatment, recovery, or discharge plan.
• Provide crisis intervention and utilize appropriate de-escalation techniques when needed.

Required Qualifications

Education

• Master’s Degree in Social Work (MSW) from an accredited school of social work.

Licensure

• Current New York State Licensed Master Social Worker (LMSW) license.

Experience

• 1–3 years of relevant social work experience required.
• Experience in a healthcare setting preferred.
• Experience in specialty areas such as dialysis, rehabilitation, substance use/ETOH, home care, oncology, or hospice preferred.
• Experience with discharge and transitional planning.
• Knowledge of community resources, entitlement programs, and eligibility requirements.
• Strong counseling, crisis intervention, and de-escalation skills.
• Experience providing psychological and emotional support to patients and families.
• Knowledge and experience identifying, assessing, and reporting suspected abuse cases.

Additional Requirements

• SBIRT certification required or must be obtained as required by the organization.
• NYS DOH Screen Certification required or must be obtained as required by the organization.

Salary Range

USD $52.08 - USD $52.08 /Hr.
