St. Francis Hospital, The Heart Center, part of Catholic Health, seeks an LMSW to provide comprehensive social work services to patients and families, assisting with emotional, psychosocial, financial, and discharge needs.

You will work with the interdisciplinary care team to assess needs, offer counseling, coordinate post-acute services, and connect patients with community resources. Requires NY LMSW license, MSW degree, and 1–3 years of hospital social work experience; SBIRT and NYS DOH Screen

Qualifications

Responsibilities

  • Provide psychosocial assessments, counseling, and emotional support to patients and families.
  • Assist with discharge and transitional planning and coordinate post-acute services and community resources.
  • Educate patients and families on financial assistance programs, benefits, and entitlements.
  • Identify psychosocial barriers and apply crisis intervention and de-escalation techniques as needed.
  • Provide psychological and emotional support to patients and families and document interactions.

Job Details

The Licensed Masters of Social Worker (LMSW) provides comprehensive social work services to patients and their families, helping them navigate emotional, psychosocial, financial, and discharge-related needs. The LMSW collaborates with the interdisciplinary care team to assess patient needs, provide appropriate support and counseling, and connect patients and families with community resources.

Key Responsibilities
  • Provide psychosocial assessments, counseling, and emotional support to patients and families.
  • Assist patients and families with discharge and transitional planning, including coordination of appropriate post-acute services and community resources.
  • Educate patients and families regarding available financial assistance programs, benefits, entitlements, and community resources.
  • Identify and address psychosocial barriers that may impact a patient’s treatment, recovery, or discharge plan.
  • Provide crisis intervention and utilize appropriate de-escalation techniques when needed.
Required Qualifications
Education
  • Master’s Degree in Social Work (MSW) from an accredited school of social work.
Licensure
  • Current New York State Licensed Master Social Worker (LMSW) license.
Experience
  • 1–3 years of relevant social work experience required.
  • Experience in a healthcare setting preferred.
  • Experience in specialty areas such as dialysis, rehabilitation, substance use/ETOH, home care, oncology, or hospice preferred.
  • Experience with discharge and transitional planning.
  • Knowledge of community resources, entitlement programs, and eligibility requirements.
  • Strong counseling, crisis intervention, and de-escalation skills.
  • Experience providing psychological and emotional support to patients and families.
  • Knowledge and experience identifying, assessing, and reporting suspected abuse cases.
Additional Requirements
  • SBIRT certification required or must be obtained as required by the organization.
  • NYS DOH Screen Certification required or must be obtained as required by the organization.
Salary Range

USD $41.33 - USD $56.76 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
