Corewell Health is seeking a Part-time Social Worker to collaborate with an interdisciplinary team and assist patients and families with complex psychosocial needs. You will coordinate discharge planning, assess safety and readiness for transition, and connect families with community resources.

Required MSW with Michigan licensure; 2+ years’ health-care experience preferred. Responsibilities include documenting plans, consultations with the care team, and maintaining regulatory knowledge.

• Required Master’s Degree in Social Work (MSW).
• 2 years of health-care related experience post-MSW preferred.
• Care management experience preferred.
• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• LIC-Master Social Worker (MSW) – Michigan Upon Hire required.
• LIC-Driver’s License – Michigan Upon Hire required.

Responsabilidades

• Conduct psychosocial assessments with patient and family to identify needs and plan care.
• Coordinate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the electronic medical record reflecting current condition and transition needs.
• Provide consultation and resources to healthcare team members.
• Maintain current knowledge of community resources.
• Participate in department, regional, or statewide projects as requested.
• Develop and maintain knowledge of federal and state regulations relevant to the role.
• Provide short term therapeutic support as appropriate for setting and location.

Job Summary

To work collaboratively with members of an interdisciplinary team in order to assist patients and families with complex psychosocial needs. These interventions may include, but are not exclusive to: alternate site coordination, discharge planning, addressing all forms of identified abuse and or neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).

Essential Functions

• Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan.
• Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient's needs.
• Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient's current condition, situational factors, transition of care needs and psychosocial imperatives.
• Provides consultation and resources to members of the healthcare team.
• Responsible for maintaining relevant and current knowledge of community resources.
• Participates or assists in department, regional, or statewide stakeholder committees, projects etc., as assigned/requested.
• Develops and maintains current knowledge of federal and state regulations as they pertain to role.
• Provide short term therapeutic support as appropriate for setting and location.

Qualifications

• Required Master's Degree Social Work
• 2 years of relevant experience health care related experience post obtaining Master's Degree in Social Work Preferred
• Care management experience Preferred
• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred Preferred
• LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan Upon Hire required
• LIC-Driver's License - STATE_MI State of Michigan Hospice and Home Care Upon Hire required
