Job summary

Liquid Personnel is recruiting a Social Worker for its Learning Disabilities Team in Somerset. You will work with adults with learning disabilities and their carers, delivering strengths-based, person-centred social work interventions to promote independence and wellbeing.

You will manage a varied caseload, undertake assessments, care planning, safeguarding, and risk management, working with health professionals and partner agencies to tailor support to individual needs and promote community

Qualifications

  • MA/BA/BSc in Social Work or CQSW.
  • Registered with Social Work England.
  • Significant post-qualified experience in Adult Social Care.
  • Strong understanding of Care Act 2014, MCA 2005 and Adult Safeguarding guidance.
  • Experience supporting adults with learning disabilities.
  • Ability to undertake strengths-based assessments, risk assessments, reviews, and person-centred plans.
  • Experience multi-agency working.
  • Driving licence desirable.

Responsibilities

  • Manage caseload of adults with learning disabilities, assess referrals and varying complexity.
  • Undertake strengths-based assessments reflecting changing needs and risks.
  • Complete Care Act assessments, reviews and support planning.
  • Manage risk and safeguard individuals from abuse or neglect.
  • Collaborate with individuals, families, carers, and partner agencies.
  • Engage with reluctant service users and support those at risk.
  • Support people during crises using appropriate legislation and resources.
  • Undertake carers' assessments and develop carer support plans.
  • Develop, implement and review person-centred care and support plans.
  • Coordinate services to meet assessed needs and negotiate care provisions.

Liquid Personnel is recruiting Social Worker for its client's Learning Disabilities Team at Somerset.

This is an exciting opportunity for a dedicated practitioner to work with adults with learning disabilities and their carers, delivering strengths-based, person-centred social work interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will manage a varied caseload of adults with learning disabilities, undertaking assessments, reviews, care planning, safeguarding interventions, and risk management. You will work closely with individuals, families, carers, health professionals, and partner agencies to ensure people receive appropriate support and services tailored to their needs, whilst promoting choice, control, and community inclusion.

What will your responsibilities be?
  • Managing a caseload of adults with learning disabilities, responding to referrals and undertaking assessments of varying complexity.
  • Undertaking ongoing strengths-based assessments to reflect changing circumstances, risks, eligible needs, and individual outcomes.
  • Completing Care Act assessments, statutory reviews, and support planning in accordance with relevant legislation, policies, and procedures.
  • Assessing and managing risk to ensure individuals are safeguarded from abuse, neglect, self-neglect, and significant harm.
  • Working with individuals, carers, families, and partner agencies to identify needs and agree appropriate levels of support and intervention.
  • Engaging with individuals who may be reluctant to access services and working proactively with those who present risks to themselves or others.
  • Supporting people during periods of crisis, using appropriate legislation, resources, and professional judgement to achieve timely resolutions.
  • Undertaking carers' assessments and developing carer support plans to ensure carers' needs are identified and addressed appropriately.
  • Developing, implementing, and reviewing person-centred care and support plans that promote independence, wellbeing, and community inclusion.
  • Arranging and coordinating services with social care, health services, and independent and voluntary sector providers to meet assessed needs.
  • Negotiating and arranging care provision, resources, and support packages to achieve agreed outcomes.
  • Preparing and presenting assessments, reports, statements, and supporting documentation to inform decision-making and care planning.
  • Maintaining accurate and up-to-date case records in accordance with legislative, professional, and organisational standards.
  • Working collaboratively with health professionals, providers, families, carers, and partner organisations to deliver coordinated and effective support.
Qualifications & Experience:

To be successful in this role you must have,

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care, preferably within Learning Disabilities Services.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
  • Learning Disabilities Experience: Experience supporting adults with learning disabilities, complex needs, and challenging circumstances.
  • Assessment Skills: Proven ability to undertake strengths-based assessments, risk assessments, reviews, and person-centred support planning.
  • Safeguarding Experience: Experience identifying, managing, and responding to safeguarding concerns.
  • Multi-Agency Working: Ability to work effectively with health colleagues, providers, carers, families, and other partner agencies.
  • Communication Skills: Excellent communication, report-writing, analytical, and decision-making skills.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
