Description:The position requires a Master of Social Work (MSW) and a current Washington license.The candidate must have experience in an acute care setting and previous travel experience.Epic experience is required, and AIDA EMR experience is preferred.The shift is 5 days a week from 07:30 to 16:00, with every other weekend required.Candidates should have experience in discharge planning and care coordination.There is a strong preference for candidates with diverse case management experience including medical-surgical and low length of stay.The average caseload is 18-24 patients during the week, increasing to up to 30 patients on weekends.The facility is a Level II Trauma Center with a total of 571 staffed beds.An interdisciplinary support system is in place, including interpretation services.The role includes conducting initial evaluations, coordinating services for home care and equipment following discharge, and documenting care delivery plans.Candidates must be flexible and may need to float between different units within the facility.Travelers' refusal to float may lead to termination of assignment.
