Description:Licensed Clinical Social Worker is a fully-qualified professional social work position.This role requires substantial professional responsibility and independence in providing social work services.Members of a multidisciplinary health care team provide psychosocial interventions to patients and their families.Work addresses a variety of biopsychosocial issues and includes assessing social needs related to health status.Developing treatment plans includes advocacy within the organization and community, counseling, referrals, and practical assistance.Consultation, orientation, and training are provided to health care staff on the effects of biopsychosocial, cultural, and economic issues.Advocacy for social work core values and ethics is encouraged within the community.May provide clinical supervision to MSWs and student interns as assigned.Prepares and maintains electronic medical records in accordance with department policies.Conducts psychosocial assessments of patients and families upon admission and as clinically indicated.Identifies and addresses barriers to care.Collaborates with interdisciplinary health care teams to devise and implement psychosocial treatment plans.Coordinates referrals to specialty and social services, recording progress notes and ensuring continuity of care.Helps patients and families cope with environmental and psychological issues of illness.Manages an independently assigned caseload of varying clinical acuity.Engages in case finding for patients and families with potential psychosocial needs.Attends team conferences and participates in interdisciplinary rounds as required.Provides supportive counseling, grief counseling, crisis intervention, and other therapeutic services.Delivers psychoeducation to patients and families regarding adaptation to diagnoses, treatments, and prognosis.Assists patients and families in the transition from youth to adult services.Participates in quality assurance and community improvement related to social work services.Contributes to the development of written policies, procedures, or guidelines related to social work services.
