Qualifications

Responsibilities

• Participates as an integral member of the Cecil County Mobile Health and Social Work teams to address patient needs, barriers to care, and community resources.
• Uses electronic medical records and population health platforms to plan longitudinal support and self-management for patients and families.
• Provides information on social service programs and resources, and coordinates care plans including DME, home health referrals, and post-acute services.
• Maintains timely documentation in patient charts and departmental records and participates in team meetings and performance indicators.
