Qualifications

• Master’s degree in Social Work or Master of Social Services from an accredited graduate school.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.
• CCM certification preferred within 18 months of eligibility.

Responsibilities

• RN Case Manager manages patient care and discharge planning.
• Social Worker resolves psycho-social barriers and supports discharge needs.
• Utilization Management reviews patient status for appropriateness and payer coverage.
