Job summary

ChristianaCare is seeking an energetic, motivated Social Worker to join our care coordination team. This role emphasizes compassionate care, collaboration across inpatient and outpatient services, and a commitment to Love and Excellence.

A Master’s degree in Social Work is required, along with appropriate Delaware licensure. Acute care experience is preferred. The position offers a $15,000 sign-on bonus to be paid within the first year of a two-year commitment.

Qualifications

• Master’s degree in Social Work from an accredited graduate school.
• Licensed CSW or MSW as required by the state of Delaware.
• Acute Care Social Work experience preferred.

Responsibilities

• RN Case Manager – manage patient care and discharge planning.
• Social Worker – resolve psycho-social barriers and supports discharge needs.
• Utilization Management – review patient status for payer coverage.
