Job summary

TriHealth in Cincinnati, OH is seeking a Social Worker in the Care Management Department of the Emergency Department. You will assess psychosocial needs, develop and implement a comprehensive plan, and collaborate with a multidisciplinary team to optimize patient flow and transitions in care.

Qualifications

• Master’s Degree in Social Science or Social Work is required.
• Licensed Social Worker is required.
• Computer literate and proficient with basic software tools.

Responsibilities

• Assess the psychosocial needs of the patient and family and develop a comprehensive care plan.
• Collaborate with the multidisciplinary team to ensure effective, appropriate care and smooth transitions between levels of care.
• Provide education and support to promote the practice of social work within the hospital and serve as a resource/preceptor for new staff.
