Responsible for the provision of social work/discharge planning services to patients in need of these services. Works in active partnership with the RN care manager, the disease manager and other disciplines to develop and implement a well-coordinated personalized plan to address patient needs across the continuum. The social worker assesses, plans and facilitates options and services to meet an individual's unique and complex needs. This is accomplished by addressing both the individual patient’s medical and psychosocial needs, as well as the resources available. There is a continuing commitment to maximizing the quality of life for each patient and enhancing community health. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values.
