Job summary

Luminis Health, Inc. seeks a Licensed Master Social Worker to provide counseling, crisis intervention, and transition planning for patients and families at the AAMC care setting. You will perform psychosocial assessments and develop service plans with the care team to support safe transitions.

The role requires an MSW and LMSW license, at least six months of healthcare experience, and a commitment to patient- and family-centered care with accurate documentation.

Qualifications

• Master of Social Work degree required.
• Six months healthcare delivery system experience required.

Responsibilities

• Provides comprehensive psychosocial assessments for patients and families.
• Counsels patients and families on adjustment to illness, discharge planning, and care needs.
• Collaborates with Care Coordinator and interdisciplinary team on transition of care strategies.
• Identifies and links community resources for patients and families.
• Documents interventions in the medical record per department standards.
• Participates in care management process improvements and rounds.
