Qualifications

Responsibilities

• Provides comprehensive psychosocial assessments for patients on case load and creates a therapeutic framework with patient and family.
• Collaborates with Care Coordinator to provide transition of care strategies in complex situations.
• Identifies and links patients to community services and resources.
• Collaborates with the interdisciplinary team and communicates patient progress and plans.
• Integrates medical and nursing information in assessment and interventions.
• Documents interventions in the patient’s medical record and participates in process improvement.
