Job summary

Prime Healthcare’s St. Mary's Hospital - Kankakee is seeking a Social Work professional to provide emotional support, crisis intervention and counseling to patients and families during hospitalization.

The role includes case management, discharge planning, and coordination with a multidisciplinary team. You will conduct psychosocial assessments, assist in treatment plan development, document care, and help connect patients with financial assistance and supportive services.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Current internship registration with the licensing board as applicable.
• Minimum one year inpatient or outpatient behavioral health experience preferred.
• Masters in related field preferred.

Department: - SMKIL Social WorkShift: Full-Time, Days, 1.0 FTEFacility: St. Mary's Hospital-KankakeeLocation: 500 W Court St, Kankakee, IL 60901Compensation Range: $28.42 to $41.17

Responsibilities

To provide emotional support, crisis intervention and counseling; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

Required qualifications:

1. Bachelor's degree Social Work, Marriage and Family Therapy or other related required.

2. Current registration an intern with Board., as appropriate to education and licensing requirements.

3. Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience preferred.

Preferred qualifications:

1. Current BCLS certificate upon hire and maintain current preferred.

Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Councilor or related field, Preferred.
