Position Overview

The Social Worker is responsible for assessing, planning, coordinating, and overseeing services for patients within their healthcare plan. The role involves managing services for patients with complex issues related to social determinants of health and psychosocial challenges to promote quality outcomes throughout the care continuum. The position ensures compliance with health plan requirements, Medicare guidelines, and standards. It acts as a liaison to coordinate care with physicians, families, and providers, advocating for patients and working as part of an interdisciplinary team.

If located in Dallas, TX, you will have the flexibility to work remotely* while tackling challenging cases.

Primary Responsibilities
  • Perform comprehensive evaluations and document findings in compliance with CMS standards.
  • Assess patients in various settings, including homes, clinics, hospitals, and nursing facilities.
  • Ensure services align with the patient’s plan eligibility and coordinate needs using community resources.
  • Collaborate with internal and external teams to improve patient and provider satisfaction.
  • Participate in Performance Improvement initiatives and team meetings.
  • Adhere to organizational policies, maintain professionalism, and uphold WellMed Core Values.
  • Maintain licensure, certifications, and ongoing training.
  • Stay informed about current health plan benefits.
  • Perform additional duties as assigned.

Required Qualifications
  • Bachelor of Social Work with 3+ years of experience beyond the required years.
  • 3+ years of social work experience in healthcare.
  • Proficiency in MS Office Suite and departmental applications.
  • Excellent oral and written communication skills.
  • Strong organizational skills and ability to multitask.
  • Willingness to travel approximately 10% to patient homes.
  • Must complete Tuberculosis screening and provide proof of immunity to certain diseases.
Preferred Qualifications
  • Master’s or Bachelor’s of Social Work with licensure (LMSW, LCSW, LBSW).
  • Experience with geriatric, behavioral, or mental health populations.
  • Long-term care experience.
  • Knowledge of disease management, managed care, and medical coding.
  • Bilingual in English and Spanish.
Physical & Mental Requirements
  • Ability to lift up to 10 pounds.
  • Ability to push/pull up to 25 pounds.
  • Extended sitting and standing.
  • Use of fine motor skills for office equipment.
  • Ability to operate a vehicle and receive instructions.

The salary range is $49,300 to $96,400 annually, based on various factors including experience, education, and location. Benefits include a comprehensive package, incentives, recognition programs, stock purchase options, and 401k contributions.
