The Social Worker is responsible for the assessment, planning, coordination, and oversight of services for identified patients within their respective healthcare plan. The position involves evaluating and managing services for patients with complex, catastrophic, SDOH (Social Determinants of Health), and/or psychosocial issues to promote quality, effective outcomes throughout the care continuum. The role ensures compliance with state and federal health plan requirements, Medicare guidelines, and standards. Acts as a liaison to coordinate and collaborate care with physicians, family, and other providers while advocating for patients, placing them at the center of care delivery. The Social Worker will interact and engage with others as an interdisciplinary team member.

Primary Responsibilities:
  1. Perform comprehensive evaluations and document findings in a concise/comprehensive manner that is compliant with documentation standards for the Center for Medicare and Medicaid Services (CMS)
  2. Perform patient assessments in multiple settings, including but not limited to the patient's home, the PCP/specialists' clinic, hospital, and/or skilled nursing facility as needed
  3. Understand and plan to ensure services provided work within the boundaries of the patient's plan eligibility. Coordinate identified patient's needs utilizing federal, state, and local community resources, as available
  4. Coordinate with external and internal teams to minimize obstacles and increase patient and provider satisfaction
  5. Participate actively in Performance Improvement (PI) or regional strategic initiatives
  6. Participate in staff and Interdisciplinary Teams (IDT) meetings
  7. Adhere to organizational and departmental policies and procedures
  8. Maintain a high level of professionalism and adhere to the WellMed Core Values
  9. Take responsibility for personal and professional development
  10. Ensure licensure, certifications, and annual training are maintained and compliant
  11. Maintain knowledge of current health plan benefits
  12. Perform other duties as assigned

Required Qualifications:
  1. Bachelor of Social Work with 3+ years of experience beyond the required years of experience
  2. 3+ years social work experience in a healthcare environment
  3. Proficient in MS Office Suite, including Word, Excel, Notes, Outlook, and other departmental specific programs/applications
  4. Excellent oral and written communication skills
  5. Strong organizational skills
  6. Ability to prioritize, plan, and handle multiple tasks/demands simultaneously
  7. Travel to clinical locations of patients' homes about 10 percent of the time
  8. Must undergo Tuberculosis screening and provide proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation, vaccination, or medical diagnosis
Preferred Qualifications:
  1. Masters or Bachelor's of Social Work with licensure (LMSW, LCSW, LBSW)
  2. Experience working with geriatric, behavioral, and mental health patient populations
  3. Long Term Care experience
  4. Knowledge of disease management, managed care, medical terminology, referral process, claims, and CPT coding
  5. Bilingual (English/Spanish) proficiency
Physical & Mental Requirements:
  1. Ability to lift up to 10 pounds
  2. Ability to push or pull heavy objects using up to 25 pounds of force
  3. Ability to sit and stand for extended periods
  4. Use fine motor skills to operate office equipment and machinery
  5. Properly drive and operate a company vehicle
  6. Receive and understand instructions verbally and in writing

The salary range for this role is $49,300 to $96,400 annually, based on full-time employment and various factors including labor markets, education, experience, and certifications. UnitedHealth Group offers comprehensive benefits, incentive programs, stock purchase options, and a 401k plan.
