Transitions Hospice is seeking a Social Worker for PRN coverage in Indianapolis, IN. The role provides compassionate psychosocial support to patients and families, with a focus on grief support, care coordination, and community resource navigation.

You will conduct initial assessments, offer ongoing counseling, and assist with Advanced Directives while maintaining compliant documentation. A driving license and local travel are required, with PRN weekday/daytime schedules available.

Qualifications

• Master’s degree in social work required.
• Licensure and/or certification as required by the state.
• Minimum of 2+ years social service experience in healthcare or social services.
• Hospice or palliative care experience preferred.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Conduct psychosocial, spiritual, and pre-bereavement assessments.
• Provide ongoing emotional support and counseling to patients and families.
• Connect families with community resources to meet practical and emotional needs.
• Guide families through Advanced Directives with clarity.

What You'll Do and Our Why

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as needed
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

Skills, Knowledge and Expertise

What You Bring

• Master’s degree required. Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
• Deep comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with the ability to work with little supervision
• Valid driver's license and willingness to travel locally for patient visits
