Job summary

Heart to Heart Hospice in Indianapolis provides compassionate care to patients with life-limiting illnesses. The Social Worker will assess psychosocial, grief, financial, and community needs and offer individual counseling to patients and families.

The role includes coordinating community resources, assisting with advance directives and funeral planning, and supporting the care team. On-call duties and timely documentation are required, with equal employment opportunities.

Qualifications

• Bachelor's or Master's in social work from an accredited program.
• Current State license required in accordance with state regulations.
• Current Driver's License with reliable transportation.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, and assistance with advance directives and funeral planning.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that match education and competence.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation.
• Ensure compliance with laws, Medicare regulations, and company policies.

What You Must Have:

• Bachelors or Master’s degree in social work from a State accredited program
• Current State license required in accordance with State regulations
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

What You Will Do:

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
