Job summary

Transitions Hospice in Bay City, MI is seeking a licensed Social Worker (LMSW) to join our team full-time. You’ll manage a meaningful caseload and advocate for patients and families through the hospice journey, from intake to bereavement support.

Coverage includes Bay, Saginaw, Arenac, Midland Counties and surrounding areas. You’ll conduct psychosocial assessments, provide counseling, connect families with resources, guide advance directives, and contribute to interdisciplinary care with

Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• Licensed in-state or eligible for licensure as LMSW.
• Minimum 2+ years in health care or social services; hospice experience preferred.

Responsibilities

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments.
• Provide ongoing emotional support and counseling to patients and families.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at residence and in agency records.
• Participate in interdisciplinary team and IDG meetings to shape care plans.
• Assist with on-call duties evenings and weekends; ensure regulatory compliance.

What You'll Do and Our Why

What You’ll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as needed
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

Skills, Knowledge and Expertise

What You Bring

• Master’s degree required. Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
• Deep comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with the ability to work with little supervision
• Valid driver's license and willingness to travel locally for patient visits
