JourneyCare Hospice in Lake Forest, IL is seeking a qualified Hospice Social Worker to join a growing team. The role offers a competitive salary, immediate opening, and generous time off benefits, including a 401(k) plan with match.

You will perform psychosocial assessments, provide counseling to patients and families, and coordinate resources while maintaining thorough documentation and collaboration with the care team. An MSW and current licensure are required.

Qualifications

• MSW is required for this role.
• Current State Licensure in Social Work is required.
• Minimum of one year’s experience in health care or hospice setting.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patient and family/caregivers and staff as needed in relation to stress and coping difficulties.
• Provides accurate and timely documentation of patient services reflecting the plan of care.
• Assists with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; connects to community resources to meet patient/family needs.
• Serves as liaison between patients, families/caregivers and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
