Job summary

JourneyCare Hospice in Illinois is seeking a Hospice Social Worker to join our growing team. The role involves performing psychosocial assessments, counseling patients and families, and coordinating resources within the patient’s care plan.

On-call responsibilities are included, with a Monday–Friday schedule and on-call 2–3 times per month. The position offers competitive salaries, comprehensive benefits (medical, dental, vision), a 401(k) with match, generous time off, and a supportive team

Qualifications

  • MSW degree and current state licensure.
  • Minimum of one year experience in health care or hospice setting.
  • Valid driver’s license and proof of insurance.

Responsibilities

  • Performs psychosocial assessment of patients and families in hospice in a timely manner and ensures needs are addressed in the plan of care.
  • Provides exceptional service to patients, families, referral sources, co-workers, and other customers; assists team members as needed.
  • Counsels patients and families on stress and coping difficulties.
  • Provides accurate and timely documentation of patient services reflecting the plan of care.
  • Assists with applications for Social Security Income, Hospice, Medicaid, and Medicaid Waiver; connects with community resources.

HOURS

Monday-Friday 8:30AM-5PM - On-Call 2-3 Times Per Month

    What You’ll Do:
    • Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
    • Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
    • Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
    • Provides accurate and timely documentation of patient services to reflect the plan of care
    • Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
    • Serves as liaison between patients and families/caregivers and community agencies
    Salary Range:

    $62,000-$80,000 - Offer based on experience
