Job summary

JourneyCare in Oak Park, IL is seeking a Social Worker to perform psychosocial assessments for hospice patients and families in their homes or community settings. You will ensure needs are addressed in the plan of care and coordinate with care teams.

Responsibilities include counseling patients and families, documenting services, assisting with applications for SSI, hospice, Medicaid, and connecting to community resources. Strong licensure and 1+ year hospice experience preferred.

Qualifications

• MSW required.
• Current State Licensure in Social Work.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

What You’ll Do:

Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required

Salary Range: $62,000-$80,000 - Offer based on experience
