Responsibilities
- Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment; communicate findings to the registered nurse and other team members. Provide assessments in the patient's residence and arrange alternative plans if safety is a concern.
- Conduct social evaluations, including family dynamics, caregiver abilities, communication patterns, and risks such as suicide, neglect, or abuse. Plan and implement interventions based on findings. Counsel patients and families on stress management and coping strategies. Provide crisis intervention when necessary.
- Assess cultural needs of patients and families, including communication styles, family roles, traditions, and taboos. Educate the interdisciplinary team accordingly.
- Maintain clinical records for all patients referred to social work services.
- Educate and assist patients and families in preparing advanced directives.
- Provide information and referral services for practical and environmental needs.
- Serve as a liaison between patients, families, caregivers, and community agencies.
- Develop and maintain collaborative relationships with organization personnel and external agencies to support patient care.
- Participate in developing individualized care plans, involving patients and families, and attend interdisciplinary team meetings to address psychosocial factors affecting health and illness.
- Assist healthcare team members in understanding social and emotional factors related to health, death, and dying.
- Contribute to quality improvement initiatives and performance assessments.
- Support families and patients in planning funeral arrangements, and managing financial, legal, and healthcare decisions.
- Perform other duties as delegated by the Clinical Supervisor.
Qualifications
- Master’s degree in social work (MSW) accredited by the Council on Social Work Education, or bachelor’s degree in social work with relevant experience, meeting personnel qualifications as per §418.114(b)(3).
- At least one (1) year of healthcare experience; hospice experience preferred.
- Understanding of hospice philosophy and issues related to death and dying.
- Experience working in hospice care preferred.
- Strong verbal and written communication skills, with good organizational abilities.
- Current licensure to practice in the state of operation, according to Medicare guidelines.
- Valid driver’s license, insured vehicle, and good driving record.
