Responsibilities
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness, communicate findings to the interdisciplinary team, and provide assessments at the patient's residence or alternative plans if necessary.
  • Conduct social evaluations, including family dynamics, caregiver abilities, communication patterns, and risks such as suicide, neglect, or abuse, and plan interventions accordingly. Offer counseling and crisis intervention as needed.
  • Educate the interdisciplinary team on any cultural needs of patients and families, including communication, family roles, traditions, and taboos.
  • Maintain clinical records for all referred patients.
  • Assist patients and families in preparing advanced directives.
  • Provide information and referral services regarding practical and environmental needs.
  • Serve as liaison between patients, families, caregivers, and community agencies.
  • Develop and maintain collaborations with community resources.
  • Participate in care planning and interdisciplinary meetings, addressing psychosocial stresses impacting health and illness.
  • Support families and patients in funeral arrangements and decision-making regarding legal and healthcare responsibilities.
  • Perform other duties as delegated by the Clinical Supervisor.
Qualifications
  • Master's degree in social work (MSW) accredited by the Council on Social Work Education OR Bachelor's degree in social work with appropriate qualifications.
  • Minimum one (1) year of healthcare experience; hospice experience preferred.
  • Understanding of hospice philosophy and issues related to death and dying.
  • Experience in hospice care is preferred.
  • Strong communication and organizational skills.
  • Current licensure to practice in the state of operation, according to Medicare guidelines.
  • Valid driver's license, insured vehicle, and good driving record.
