Zusammenfassung

Hospice & Community Care is seeking a Social Worker for Home Hospice to collaborate with the Interdisciplinary Team and provide psychosocial support to patients and families.

The role requires MSW and licensure within two years, covering Lancaster and surrounding counties, with an orientation period of 4–5 weeks. Per diem, day shift, 64 hours per two weeks, with community-based responsibilities.

Social Worker - Home Hospice

This role works in collaboration with the Interdisciplinary Team to provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers.

MSW required. LSW or LCSW must be obtained within two years from the date of hire. This position covers Lancaster, York and surrounding counties. Must be able to complete 4–5 weeks of a full‑time day‑shift orientation.

Per Diem – as needed. Day Shift (8:00 am – 5:00 pm). Must work a minimum of 64 hours per two‑week pay period.
