Job summary

Hospice & Community Care is seeking a Social Worker for Home Hospice to collaborate with the Interdisciplinary Team and provide psychosocial support to patients and families.

The role requires MSW and licensure within two years, covering Lancaster and surrounding counties, with an orientation period of 4–5 weeks. Per diem, day shift, 64 hours per two weeks, with community-based responsibilities.

Qualifications

• Position covers Lancaster, York and surrounding counties.
• Must complete 4–5 weeks of full‑time day‑shift orientation.

Social Worker - Home Hospice

Per Diem – as needed. Day Shift (8:00 am – 5:00 pm). Must work a minimum of 64 hours per two‑week pay period.
