Job summary

Vitality Hospice is seeking a dedicated Hospice Social Worker (MSW) to join a compassionate team delivering comfort, dignity, and support to patients and families in homes and community settings.

You will provide psychosocial support, conduct assessments, develop care plans, guide end-of-life planning, and coordinate care with the interdisciplinary team. Travel to patient residences is required; per diem scheduling and ongoing professional growth are available.

Qualifications

• Current MSW (required).
• Hospice or healthcare experience preferred, but compassionate professionals from other settings are welcome to apply.
• Excellent communication, empathy, and problem-solving skills.
• A heart for serving patients and families at a meaningful time in their lives.

Responsibilities

What You’ll Do
