Job summary

Lynn Community Health Center is seeking a Social Worker for the One Care program to address the social service needs of clients. This role involves providing follow-up sessions, conducting home visits, and advocating for patients with outside agencies.

The successful applicant will have a minimum of a bachelor’s degree in human services, with preference for bilingual candidates and a focus on maintaining a positive work attitude.

Qualifications

• Minimum of a bachelor’s degree in the human service field (e.g., psychology, sociology) required.
• Bilingual/bicultural candidates strongly desired.
• History of good attendance and a positive work attitude.

Responsibilities

• Address the social service needs of One Care members.
• Provide follow-up sessions with clients or coordinate services by phone.
• Conduct home visits with interdisciplinary team members.
• Advocate for patients with outside agencies.
• Participate in case conferences as needed.
