The ideal candidate holds a Bachelor’s in Social Work or related field, with Iowa licensure preferred and CPR certification beneficial.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field required.
• One year of supervised social work experience in a health care setting preferred.
• Iowa Board of Social Work licensed preferred.
• CPR Certification preferred.

Responsibilities

• Confers with resident and family to develop a psychosocial assessment and care plan.
• Orients residents and families to facility services and rights.
• Completes MDS sections and PASRR screening as needed.
• Participates in care plan conferences with residents, families, and staff.
