Friendly Faces Senior Care is seeking a compassionate Social Worker to assess client needs, develop care plans, and provide counseling across hospital, clinic, school, or community settings. The role supports diverse populations and emphasizes advocacy, collaboration, and ethical documentation.

The ideal candidate holds an MSW with optional LMSW/LCSW credentials and strong interpersonal and written communication skills.

Qualifications

• MSW from an accredited program is required; LMSW or LCSW preferred but not mandatory for all positions depending on role requirements.
• Previous experience working with diverse populations is highly desirable; new graduates are encouraged to apply for certain positions
• Strong interpersonal skills conducive to building rapport with clients
• Excellent written communication abilities

Responsibilities

• Conduct comprehensive assessments of clients' needs, strengths, and challenges.
• Develop individualized service plans that address client needs effectively.
• Provide counseling support to clients dealing with mental health concerns, family dynamics, crisis intervention, or substance abuse.
• Advocate for clients' rights within healthcare settings or community resources.
• Coordinate services with other professionals including healthcare providers, governmental agencies, schools, and community organizations.
• Maintain accurate records of client interactions including assessments, case notes, treatment plans in compliance with ethical guidelines and legal regulations.
• Stay informed about local resources available for clients to provide appropriate referrals.
• Participate in multidisciplinary team meetings to discuss patient care strategies.
