Job summary

Norman County Social Service Center is seeking a full-time County Agency Social Worker to deliver social work services and manage a caseload focused on adults with disabilities, older adults, and vulnerable adults in Minnesota.

You will assess needs, coordinate care, and administer state/federal programs, while collaborating with families, foster care providers, health plans, and state agencies. The position offers a 35-hour workweek and comprehensive benefits.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field.
• Or bachelor's degree in any field with 1 year of social worker experience.

Responsibilities

• Perform assessment, case management, care coordination for adults with disabilities, older adults, and vulnerable adults.
• Provide adult and child protective services, and employment services.
• Administer state/federal programs (home/community waivers, MN-Choices, Obra Level II, etc.).
• Coordinate services with families, foster care providers, health plans, and state agencies.
