Job Summary

Provides Disease Management (DM) services to TRICARE beneficiaries. The Social Worker – Disease Management collaborates with the clinical team to assess, plan, implement, coordinate, monitor, and evaluate the beneficiary’s health and psychosocial needs, focusing on empowering the beneficiary to manage their medical, behavioral, and psychosocial needs related to their chronic condition. The SW partners with the interdisciplinary care team to identify appropriate care to prevent complications and minimize exacerbations using evidence-based tools. The role involves contributing to care planning activities and assessing outcomes to improve health status.

Education & Experience

Required:

• Current, unrestricted SW licensure (BSW) in the state of residence
• U.S. Citizenship
• Ability to pass a favorable DoD background investigation
• Minimum two years’ experience in case management, care coordination, or discharge planning in healthcare or MCO settings

Preferred:

• Experience with MCO in Care or Disease Management
• Master’s degree in Social Work (MSW), LCSW, LMSW
• Knowledge of government healthcare programs, TRICARE, Medicare/Medicaid

• Assess beneficiaries’ needs to support care goals
• Collaborate on whole-person care plans incorporating assessment findings
• Provide education and coaching for behavioral and psychosocial needs
• Manage activities for appropriate resource utilization and access
• Identify and facilitate resources related to health disparities
• Develop measurable outcomes and monitor program impact
• Assess high-risk factors and needs for additional services
• Support beneficiary transfers between regions with military liaison collaboration
• Follow URAC standards, TRICARE policies, and practice standards

Competencies

Includes communication skills, computer literacy, flexibility, empathy, independent thinking, multi-tasking, organizational skills, problem-solving, team-building, and technical knowledge of TRICARE and case management standards.

Working Conditions

• Ability to cover any shift and work overtime if needed
• Onsite or remote work with a secure workspace and high-speed internet
