Job Summary
Provides Disease Management (DM) services to TRICARE beneficiaries. The Social Worker – Disease Management collaborates with the clinical team to assess, plan, implement, coordinate, monitor, and evaluate the beneficiary’s health and psychosocial needs, focusing on empowering the beneficiary to manage their medical, behavioral, and psychosocial needs related to their chronic condition. The SW partners with the interdisciplinary care team to identify appropriate care to prevent complications and minimize exacerbations using evidence-based tools. The role involves contributing to care planning activities and assessing outcomes to improve health status.
